World War I was fought primarily from entrenched positions. Soldiers from opposing armies dug deep, complicated fortifications on either side of a barren "no man's land," battling back and forth over a few square miles of territory for months at a time. "Over the top" meant an attack: soldiers would have to climb up out of their trenches, over the top of the barricades, in order to charge at the enemy's side of the line. Going "over the top" was a harrowing and extremely deadly experience.
